Relapse Prevention Planning
Supportive Planning to Help You Stay Prepared and Connected
Relapse Prevention Planning at The Blooming Path offers calm, peer led support to help you build awareness, prepare for challenges, and stay connected to your recovery goals. These one on one sessions provide a supportive space to reflect, plan, and strengthen strategies that support stability and forward movement.
Relapse does not mean failure. Having a thoughtful, realistic plan can help you recognize warning signs early and respond with care and intention.
Understanding the Relapse Prevention Planning Approach
Relapse Prevention Planning is grounded in lived experience and focused on preparation and self awareness rather than instruction or treatment. Sessions emphasize collaboration, strengths, and practical planning, not diagnosis or clinical intervention.
The approach is participant led and respects individual pace. Planning is shaped around what feels most relevant to your experiences, needs, and goals.
What This Service Is

Support may center on identifying risk patterns, strengthening coping strategies, and clarifying steps to take when support is needed. The focus is on creating a plan that feels realistic, empowering, and usable in everyday life.
